Abstract
We present a case of retrosternal goiter with dilated external jugular veins which points to retrosternal extension. This case highlights the importance of clinical examination even in this era of imaging since vital clues can be elicited. This case also emphasizes the fact that some clinical signs and manoeuvres may be uncomfortable and can result in distress to the patient and one such sign is Permberton¡¯s sign and better avoided during clinical examination.
